Dual left anterior descending artery (LAD) is a rare coronary anomaly with varied presentation. We present the case of a 63‑year‑old male with complaints of exertional angina whose coronary angiogram and computerized tomography coronary angiogram showed

type 4 dual LAD arteries, a rare variant of dual LAD.
